Return-to-Duty & Follow-Up Testing — What We Provide
ExpressCare Labs provides the collection service. Violation management decisions are made by the employer and SAP.

Return-to-Duty (RTD) Testing

After a DOT violation, a SAP will authorize when an employee is ready for an RTD test. We provide the certified collection and submit results to your MRO as required. A negative result is required before the employee may resume safety-sensitive functions.

Follow-Up Testing

Federal regulations require a minimum of 6 unannounced follow-up tests in the 12 months following RTD, with the program continuing for up to 5 years as directed by the SAP. We provide collection services on the schedule your compliance program requires.

Common questions

Employer FAQ

QWhat are the 2026 DOT random testing rates?

FMCSA: 50% drug / 10% alcohol. FTA: 50% drug / 10% alcohol. PHMSA: 50% drug. FAA: 25% drug / 10% alcohol. FRA: 25% drug / 10% alcohol. Source: DOT ODAPC, effective January 1, 2026. Employers should consult their compliance officer or TPA for program-specific guidance.

QIs a consortium required for owner-operators?

Federal regulations require CDL drivers and owner-operators to be enrolled in a consortium or standalone random testing pool. A rule of thumb: fewer than 20 drivers typically use a consortium. We can help determine the right structure for your situation.

QWhat is chain-of-custody and why does it matter?

Chain-of-custody is the documented process from collection to lab analysis that ensures sample integrity. It makes results legally defensible in audits, disputes, and compliance reviews. All DOT collections require it.

QDo non-DOT employers need drug testing?

Non-DOT testing is not federally mandated for most employers, but many construction, manufacturing, healthcare, security, and warehouse companies maintain programs for workplace safety, liability management, and insurance requirements. We support all of these.

QWhat labs process the tests?

Quest Diagnostics and LabCorp — both SAMHSA-certified under the HHS National Laboratory Certification Program. These are the same labs used by the industry's largest TPA providers.

QDo you offer TB and immunity testing for healthcare employers?

Yes. QuantiFERON Gold TB blood testing, MMR titers, and Varicella titers are available for pre-employment and annual compliance. We support healthcare facilities, staffing agencies, and school districts.
